Terms and Conditions

Failure to cancel within 12 hours of appointment results in cancellation fee of 50% of service

No call no show results in cancellation fee of 50% of service

More than 15 minutes late will require appointment reschedule and fee of 25% of service

Will not travel for services that don’t amount to 60 min

All travel clients are required to book one appointment in office as screening/consultation

Any unprofessional situation occurring during home visitation will result in immediate termination of client

Will travel to Big Sky for travel fee of $30 (April-October) or $50 (November-March)

All minors are required a consent form signed by a parent or legal guardian

Minors under 16 must be accompanied by parent or legal guardian

Verbal consent and conversation regarding scope of practice and expectations will occur before each service involving a minor with legal guardian present
